Label your tuners as H1, H2, S1 and S2. (H for hybrid, S for satellite).
For example,
if H1 fails whether you put it in Tuner Slot A, Tuner slot B, Tuner Slot C or Tuner Slot D, we can conclude there is a problem with Tuner module H1.
On the other hand,
if you put H1, H2, S1, or S2 in Tuner Slot C, and you always havea problem with it in Slot C, we can establish there is an issue with the slot/arrangement.

Sounds to me like the slot for tuner D might be the problem as that is the slot that the tuner us in which works and the disappears as the OP has mentioned above.

I have tried in all slots today, still no joy
no just 1 sat tuner module is failing, hybrid tuners and other sat all are working fine, sponsors have asked to return the tuner

how have you set them ie 2 x dvb= a b, then hybrid = c d??
I have slots A and C with DVB-C and C and D with DVB-S. Originally had DVB-S in slots A and B but this caused issues when recording 2 cable channels at the same time.
?? do you mean A and B and C and D? ie B is missing from above!
I had a similar problem with my et 8500 and turned out to be a faulty tuner
For anybody else, I went with A = DVB, B= HYBRID C/T, C= HYBRID C/T, D= DVB all working perfectly.
