Search the forum for PVR_EPG & EPGRefresh. Lots of information already posted.

First you have to set your provider in the ABM and select your Cable area code mine is 00010 Teesside and also select virgin in the CrossEPG providers

Thats a different topic and nothing to do with EPG.
There are no plugins to do that, you will have to manually create a bouquet using the receiver or programs such as DreamBoxEdit/Dreamset on PC.
