Is there a tutorial or information on constructing PICONS?

I'm aware that they are small images that can be constructed/modified using many of the popular image programs. This is not a problem for me.

What image formats are acceptable? I see that from many of the examples here that .png is used but can, say, .gif or another image format be used?

How is the image (icon) then matched to a channel, say, a UK terrestrial Freeview channel? Again, looking at some examples here and on-line the file name appears to be the reference to the channel but whats the format for this naming convention? Would the linking be based on transmitter MUX information, and possible different for each transmitter in the UK, or on LCNs, or on something else?

They are named after service reference.
