Yes m8, seperate box ans card but it must be connected to a phone line.

they say must be connected but mine has not been connected from day one and actually come to think of it my 12 months is up or will be in a week or so

as i have said before mate, your very lucky then. mine was disconnected ( accidentally may i add ) for 2 months and i got hit with a double bill of £140 for each month and a letter saying for every month it was not connected i would get billed for two full subs.
